A homeless man with
advanced pulmonary tuberculosis presents with severe backache. His left hip
is flexed and laterally rotated, and thigh extension is very painful.
Radiography shows destruction on the left side of the anterior portions of
vertebrae T12–L1. A diagnosis of TB of the T12 IV disc and left-sided psoas
abscess is made. Explain the patient’s posture. What structures are at risk
in resection of the abscess?
